How Grace is designed. This page shows the design of the full system. The demo uses scripted messages; in the full design, the language model writes replies only after the rules below have run, and a person owns every escalation.

Every message takes the same path

1Red-flag screenFixed rules check for emergency signs first, before any AI writes a word.
2Triage levelOne of five levels, from call 911 to general information.
3Reply in plain languageDrafted from the program’s own instructions, never a diagnosis.
4Hand-offUrgent and emergency messages reach a coordinator with a summary.
5RecordMessage, level, reply and who picked it up, kept for review.

    The five triage levels

    • EmergencyCall 911 now, and the team is told at the same time. Chest pain, trouble breathing, stroke signs.
    • UrgentContact the transplant team now; a coordinator is paged. Fever above the program’s threshold, signs of infection or rejection.
    • Within 24 to 48 hoursTeam follows up soon. Missed medication, new side effects.
    • MonitorKeep watching, with clear signs that would raise the level.
    • InformationDiet, routine, appointments and how-to questions.

    What Grace never does

    • Diagnose. It triages and guides; the team diagnoses.
    • Change a medication. Dose questions always go to the team.
    • Talk someone out of calling. If in doubt, the level goes up, not down.
    • Hide a hand-off. The patient is told when a person has been alerted.

    Why it is different

    • Rules before the model. Emergencies are caught by fixed checks, not by how a model reads the message.
    • Built around the coordinator. Grace takes the routine questions so people can take the urgent ones.
    • A record for every hand-off. Who was alerted, when, and what they did.
